The above-named Player wishes to join the United Select Soccer Academy. As Parent/Guardian, I hereby give my consent for him/her to attend. I understand the course is non-refundable, unless cancelled by the organiser, on behalf of the above-named Player, I/we understand that soccer is a physical sport and that neither United Select nor any person duly authorised by it, nor the operator of any venue at which a course is being held, shall be held liable for any loss, accident and/or injury to the Player. This does not affect your Common Law rights. I understand and accept that sessions maybe cancelled due to inclement weather.
